@Orchard LLC is a subcontractor supporting the Veterans Affairs Administration with an urgent need for an experienced Medical/Surgical Registered Nurse (RN) to provide comprehensive care to patients requiring acute medical or surgical interventions at the Cobb Co. VA Health facility in Marietta Georgia. The RN will function both independently and under the guidance of the Nurse Manager, delivering a holistic approach to patient care, in an ambulatory setting. Responsibilities include patient assessments, chronic disease management, care coordination, health education, and collaboration with an interdisciplinary team to improve patient outcomes.
As the CRNA your responsibilities will include;
Patient Care & Coordination:- Complete post-Emergency Department (ED)/Urgent Care and post-discharge follow-ups within two business days, documenting results in the medical record.
- Actively manage a high-risk patient registry, including those with:
- Elevated HgbA1C, hypertension, elevated LDLs
- Frequent hospitalizations/admissions
- Frequent visits to the ED/Urgent Care
- Patients identified as high risk for injury
- Collaborate with the Primary Care Provider, RN Care Manager, Clinical Associate, Clerical Associate) and the expanded team (family/caregivers, community-based services) in developing the patient-driven care plan.
- Provide patient and family health education focused on self-management, prevention, and wellness based on the patient's personalized health plan.
- Manage triage, telephone triage, and scheduled/unscheduled patients in the outpatient setting.
- Execute nurse-driven clinics to support positive health outcomes for Veterans.
- Perform phlebotomy, point-of-care testing, and EKGs.
- Administer medications, immunizations, and treatments as prescribed.
- Screen patients for social services, home care, and other community resources, making referrals as needed.
- Provide care coordination for specialty services including cardiology, dermatology, and pain management.
- Maintain accurate and timely electronic health record (EHR) documentation.
- Communicate patient progress, issues, and concerns to providers and case managers.
- Promote cost-effective care strategies, including telehealth and non-face-to-face patient management.
- Verify and follow physician orders as needed.
- May be required to work with COVID-19-positive patients and Patients Under Investigation (PUI).
- On-call status may be required at the Nurse Manager's discretion.
- Demonstrate professionalism and excellent customer service in all interactions.
- Licensure: Current, unrestricted RN license from any U.S. state.
- Certification: Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certification accredited by the American Heart Association.
- Experience:
- Minimum three (3) years of recent primary care experience (must have worked within the last six (6) months).
- Experience managing high-risk patient registries, chronic disease management, and telehealth services preferred.
- Education:
- Bachelor’s degree in Nursing (BSN) or a related field is preferred; however, an Associate Degree or Nursing Diploma will be considered with valid licensure and relevant experience.
- A graduate of a nursing program accredited by ACEN, CCNE, or ANCC.
