A division of the City of New York seeks an Aftercare and Prevention Outreach Coordinator on a long-term, temporary basis.

Primary Duties: 

  • Provide direct assistance to ensure housing stability for formerly homeless and at-risk Veterans and Military Families through critical time intervention outreach phone calls and emails.
  • Manage the Veteran aftercare database, and other tools that support aftercare and prevention work as needed.
  • Assist in the development and maintenance of relationships with public and private partners regarding special projects and initiatives
  • Assist in the planning, implementation and support of special initiatives and projects that support current or former clients as it pertains to aftercare and prevention
  • Represent the Aftercare and Prevention Department as needed in meetings, conferences and/or events
  • Collaborate with and support internship task assignments 
  • Comply with all policies and procedures, environmental health and safety, dress code, time and leave policies, and Code of Conduct
  • All other duties assigned by supervisor


  • Possess knowledge regarding city, state, and community resources
  • Possess a basic knowledge of Veteran and Military family needs and services
  • Possesses the skills of engagement and needs assessment to mental health and social needs
