Job Summary

The Care Management Coordinator is an outreach specialist that supports the Moda Clinical Care Team by assisting in member engagement through outreach campaigns, collecting member-specific health data, documentation of member outreach/engagement, and data entry. This is a temporary WFH role.

 

Pay Range
$17.00 - $18.55 hourly, DOE. 

*Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Required Skills, Experience & Education

  • Proficient with Microsoft Office applications and type a minimum of 35 wpm on a computer keyboard.
  • Strong analytical, problem solving, memory retention, organizational and detail orientation skills.
  • Exceptional verbal and interpersonal communication skills.
  • 1+ years of experience in a patient-facing role (e.g., medical receptionist, assistant, or care coordinator) and familiarity with basic medical terminology preferred.
  • Demonstrates professionalism, empathy, and discretion when working with patients, caregivers, and medical staff; adheres to HIPAA regulations.
  • Ability to work well under pressure.
  • Project a professional business image telephonically and in person.
  • Ability to maintain excellent attendance.


Primary Functions

  • Initiates mailing campaigns to encourage case management engagement.
  • Collect information about a member’s social, clinical, and functional status to identify individual and case management needs.
  • Create, enter, and complete Health Risk Assessments and initiate appropriate care plans within the members health record.
  • Collaborate with RN Case Managers to initiate referrals, communicate specific member needs, and to assist in care planning.
  • Coordinate and initiate referrals to appropriate services and programs utilizing a large network of resources to assist member needs
  • Provide consistent, accurate and timely documentation.
  • Plan, organize, and prioritize assignments to comply with performance standards, corporate goals, and established timelines.
  • Performs outreach calls to inform patients about care coordination services and start case workflows.
  • Other duties and responsibilities as assigned.


Contact with others & Working Conditions

  • Prolonged amounts of sitting and working on PC with extensive keyboard and telephonic work daily. 
  • Initial orientation is performed in an office setting at Moda Tower, Portland, Oregon.  Then transition to work from home/remote upon successful orientation.
