POSITION TITLE: Case Manager – ILP                                                       FLSA: Non-Exempt

Salary Range $23- $25 an Hour
OVERVIEW: 
Serve Independent Living Program (ILP) youth and young adults as a teacher, mentor and guide to assist them with their Independent Living Skills needs. Function within the program through assisting with training, functioning as a liaison, and with program development and service enhancements.

As with all UC staff, this position is responsible for adhering to the performance standards set forth in the UC Staff Employee and UC policies and procedures.

QUALIFICATIONS:
Required:
  1. Bachelor’s degree in Behavioral or Social Sciences required.
  2. Minimum of 3 years working with emotionally disturbed children/adolescents/young adults.
  3. Knowledge/skills/abilities in transition age youth unique needs and services.
  4. Knowledge/skills/abilities in ILP program service delivery.
  5. Must meet Community Care Licensing Requirements for employment.
  6. Knowledge of Microsoft Offices applications.

RESPONSIBILITIES INCLUDE (but not limited to):
  1. Field internal and external referrals for youth to be engaged with the ILP program.
  2. Complete necessary documentation to include: assessment, intake, generating reports (monthly, quarterly, and annual), session write-ups, tracking outcomes, data entry into computerized charting system, keeping paper-based charts, updating Step-Transitional Independent Living Plan (TILP) for emancipated youth, tracking programmatic expenditures. 
  3. Provide service activities to both in-care and emancipated youth, consistent with their TILP in a 1:1 setting, group setting, or both.
  4. Provide ancillary individual case management for the youth focusing on their life skill needs to include education, employment, life skills.  This case management is not meant to take the place of any other case management they are/should be receiving (such as from group home agency, or THP provider), but as a supplement.
  5. Be available and attend youths’ family team meetings and emancipation conferences to provide feedback, support and resources related to the youth’s upcoming transition into adulthood.
  6. Attend ILP meetings and conferences.
  7. Represents the agency, as needed, in meetings with outside agencies, community groups and organizations; fosters collaborative working relationships to the benefit of the programs and the agency.  Collaborate and coordinate with other agencies and community resources to better service the youth.
  8. Provide needed transportation to enable a youth to participate in ILP related events if it fits within their TILP.
  9. Assist and guide youth in locating/navigating educational opportunities, vocational training, and employment opportunities.
  10. Distribute financial incentives/assistance to the youth in the form of incentives and/or emancipated youth stipend.  Maintain records of all distributions.
  11. Evaluate and make recommendations for service delivery; assist in developing and implementing new programmatic developments.
  12. Train program staff in ILP related service delivery.
  13. Complete special projects as assigned by supervisor.
  14. Coordinate all workshop activities including but not limited to creation of annual calendar and workshop material, recruitment of guest speakers, and facilitation of workshop.
  15. Organization Duties:
    1. Timely and accurate submission of timecards to supervisor
    2. Time away from the Office, Program and/or Home (i.e. vacations, personal time off, leaves of absence) must be pre-approved by supervisor and requested based on process outlined in the UC Employee Handbook.
    3. Maintain knowledge of staff handbook, policies, procedures, manuals, and pertinent documentation
  16. Other duties as assigned.

  6. Employee is occasionally required to stand; walk; sit; use hands to handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop; kneel, crouch or crawl; talk or hear; taste or smell. 
  7. Employee must occasionally lift and/or move up to 25 pounds. 
  8. Employee may be required to make or receive physical contact with clients and residents,  such as  in the process of delivering First Aid/CPR or de-escalation interventions.
  9. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  10. Must have CPR and First Aid certificate within 10 days of start date and maintain a current certificate with Human Resources.
  11. Must clear a Department of Justice background check including fingerprinting before start date and maintain clearance.
  12. Must be able to work in a DSS Community Care Licensed Facility.
  13. Must clear a health screening, including TB before start date and maintain clearance.
  14. Must have a car, valid California Driver License and be able to drive, have a clean driving record and maintain a clean driving record.
  15. Meet and maintain the minimum vehicle liability and property insurance limits (to be insured by our insurance carrier for using vehicle for business purposes such as  transportation of residents or reimbursement of business-related mileage expenses ).
