Starting Salary: $45,000/year

Position Summary: This position provides Care Coordination/Case Management in the Parenting with Love and Limits Program and assist the PLL Therapists support the family as the family and youth work through the program. The PLL Care Coordinator will conduct client assessments, develop the recipient Service Plan and serve as the liaison with the recipient’s family to address issues related to implementation of the Service Plan. The Provider will link and facilitate service recipients with appropriate services and resources identified in the Service Plan through referrals to reach each desired goal while advocating for necessary services.

Technical/Functional Expectations:

  1. Authorization Coordination-Essential Function: This is required to ensure timely delivery of appropriate services while managing the budget allocations and identifying alternative community resources as well as other funding streams in order to maximize resources and adhere to policies and procedures. Responsible for utilization and monitoring flexible funding and service coordination in order to meet client needs.
  2. Crisis Interventions - Essential Function: Provide crisis intervention linkages, referrals through consultation to ensure family support and stabilization.
  3. Care Planning Resources: Access 3rd party, Medicaid and community resources and ensures inclusion in care planning in order to maximize resources.
  4. Solution-Focused, collaborative communication-Essential: This is required in order to build trust and rapport and to engage families and other members of the team in support of the family to identify the needs and to drive to positive outcomes for the family.
  5. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting and training others, and performing duties of higher rated positions from time to time for developmental purposes.

Business Experience:

  1. Valid Driver's License-Essential: This position requires a valid driver’s license in order to travel throughout the County.
  2. 2+ years Family Team Conferencing/Wraparound Experience: This position requires Family Centered Care Planning and Wraparound facilitation training and experience for at risk families.
  3. Computer skills (Microsoft and Web based application): This position uses Microsoft and a web based application for daily entry of case activity and the utilization management of service authorizations.
  4. 2+ years of Team Facilitation Skills: This position works collaboratively with both informal and formal professional supports and must demonstrate the ability to effectively partner with collaterals and service providers.
  5. 2 years of Case Management/Service Coordination Experience: This position will be authorizing services, managing a budget and will act a coordinator of other services pending family needs.
  6. Organization and Planning Skills: Organization and planning skills are essential in order to manage a complex case load and to ensure the timely delivery of services through a wide range of sources.

Educational and Experience Requirements:

  1. Bachelor’s Degree required.
  2. 2 to 5 years related experience required.
  3. 5 to 8 years related experience preferred.
