Description

As we continue to grow, we are seeking a Pharmacist to join our award-winning specialty pharmacy team. 

We are actively recruiting a motivated and service-oriented Specialty Pharmacist for an exciting new role supporting specialty pharmacy patients and medical providers within select hospital clinics in the Pacific Northwest. This role will center on providing services within a specific PeaceHealth out-patient medical clinic, located in Longview, WA. Through an integrative patient care model, this role will act as an in-clinic specialty pharmacy liaison who partners with other members of the clinic care team to coordinate assistance and provide support throughout each patient’s treatment journey.

As an Integrated Specialty Service Pharmacist, you will be an integral part of a medical clinic care team providing services to support patients initiating and continuing treatment with a specialty medication. The Integrated Specialty Service Pharmacist will provide in-clinic services centered on expediting patient access to specialty medication treatments and ensuring safe and effective therapy. This role will participate in optimizing treatment initiation, delivering patient education and counselling, as well as supporting the identification, prevention, and resolution of medication-related problems. The Integrated Specialty Service Pharmacist will collaborate with other members of the medical clinic care team to ensure seamless delivery of patient care services.

Position responsibilities:
  • Establishes relationship as a clinic resource with patients initiating and continuing treatment with a specialty medication.
  • Evaluates specialty pharmacy treatment selections against clinical guidelines and appropriate evidence-based literature.
  • Conducts a prospective review to ensure requirements are met for treatment initiation, coordinating follow-up as warranted.
  • Performs drug-utilization-reviews and identifies treatment adjustment as necessary.
  • Provides patient education and appropriate patient follow up per clinic protocol.
  • Evaluates referrals for accuracy, completeness, and clinical appropriateness.
  • Provides direction to Specialty Pharmacy Liaison Technicians to expedite medication access and ensure treatment affordability and adherence.
  • Provides in-person medication counseling and administration training.
  • Actively utilizes integrated and routine communication with all healthcare professionals with an emphasis placed on monitoring and documentation of specific medication therapy outcomes.

Required skills/credentials:
  • Active Washington Pharmacist license

Our ideal candidate has:
  • Strong clinical knowledge of disease state and medication management pertinent to specialty clinic practice.
  • 1-3 years’ experience in complex clinical patient management preferred.
  • One or more years related experience working in a specialty pharmacy preferred.
  • Strong communication and interpersonal skills.
  • Ability to effectively collaborate in a team setting.
  • Proficiency in standard workplace software and/or information management applications.
