Licensed Clinical Social Worker (LCSW)

Job Description:

The Licensed Clinical Social Worker (LCSW) will work with patients and their families to assess treatment needs.  The LCSW is responsible for conducting interviews and assessments, setting treatment goals and monitoring plan objectives.    

Essential job functions:

• Plans and evaluates individual service plans and monitors plan objectives.
• Performs individual, family and group counseling utilizing approved treatment approaches.
• Prepares patient progress reports and completion notices.
• Assists patients with reaching their treatments goals.
• Executes therapeutic interventions, skill building and case management for clients.
• Maintains accurate records to ensure compliance with all Federal and State regulations.
• Serves as an information resource for mental health issues.
• Consults with physicians, therapists, and other medical professionals when appropriate.
• Assess the psychological status of patients related to the patient’s illness and environment.
• Provide relevant referrals to outside agencies and service providers based upon needs.
• Generate appropriate reports for quality and metrics.

Job Requirements: 

Master’s Degree in Social Work, or equivalent, from an accredited educational institution.

Licensed in the State of Texas as a Licensed Clinical Social Worker (LCSW).

Fluency in English and Spanish. 

At least two (2) years of experience.
