TRANSPORTATION PARAPROFESSIONAL / HEALTH AIDE
Am pick up from students home at 6:45 am from Bronx zip code 10463
Dismissal time for PM drop-off at 2:40 pm from school zip code NY 10002
Para would need to be trained in signed/symptons of low and high glucose levels, how to do finger sticks to check glucose levels and how to administer glucagon. School nurse and parents would train the para/health aide.
Assignment will last duration of school year.
Requires transportation para from home to school MOnday through Friday and school to home Monday, Wednesday, and Friday (soon to change to M, Th, F)
Provider who can accommodate to this need would have a placement in close proximity to students school when student is attending educational setting.
