POSITION OBJECTIVE:   To provide support to Information & Eligibility and Data Services by being the primary point of contact for customer service, client benefits and data related issues. This is a high visibility position which will have communications internally and externally to resolve problems.  It is anticipated that all issues brought to the attention of this position require an immediate response to the originator within specified timeframes. 
 ESSENTIAL FUNCTIONS:                                                                                     

Primary point of contact for department mail received from the Agency for Health Care Administration or other providers related to client insurance or eligibility. 
Primary point of contact for Express Enrollment, the completion of Medicaid plan enrollment activities and tracking.
Primary customer service point of contact for benefit and data support issues including but not limited to Social Security card applications, Youth Credit Checks, Medicaid/PCP/HMO Issues. 
Research and resolve data services and client benefit questions and inquires being the go between for internal departments and external customers as appropriate.
Track and maintain Department of Juvenile Justice involvement for youth in care.
Complete immediate responses to inquiries acknowledging we have their inquiry and that we intend to have a solution within 24 hours. 
Maintain frequent communication with internal staff regarding changes to case data and eligibility including but not limited to IES, Records Room, Adoption, GAP, Youth Services and Operations staff. 
Accurately enters required program information adhering to strict deadlines
Obtains and maintains appropriate eligibility documentation 
Completes applications for Medicaid and Federal/State Funds
Maintains open communication with Protective Investigators and Case Managers responsible for supervising cases of children in care, obtaining information on income, household composition and family resources in order to determine eligibility
Queries systems to access specific information for distributing weekly/monthly reports to case management and senior management staff 
Participates and documents CTS staffing&rsquo;s.
Utilize established checklists, forms and task tracking tools identified in the Information & Eligibility Handbook to report work status and maintain required documentation.
Distribute and review reports and complete routine data validation activities.
Escalate high profile requests and issues.
Identify trends within department and present solutions.
Assist in training Information & Eligibility Specialists on new policies and procedure.  
Assist in Federal Funding Eligibility Monitoring Reviews.
Attends regular IES team meetings, staff meetings, service center meetings, case management meetings, in-service trainings and conferences as needed.
Participates in peer file review activities.
Special projects for the department as assigned.
Participates in the agency strategic plan & quality improvement processes. 
Ability to safely & successfully perform essential job functions consistent with the ADA, FMLA, & other federal, state, & local standards, including meeting qualitative & quantitative productivity standards.

Position Summary: The Circuit Manager of Out-of-Home Care (OOHC) is responsible for the day-to-day management and coordination of placement services within Circuit 9, serving Orange and Osceola Counties. This position ensures placement operations are conducted in accordance with contractual requirements, Florida Statutes, Florida Administrative Code, and organizational policies, while supporting the Director of Out-of-Home Care in achieving departmental goals and performance expectations.
 
The Manager directly supervises two Out-of-Home Care Supervisors, an Intake Behavioral Health Coordinator, and Out-of-Home Care Specialists responsible for coordinating licensed placements, including foster homes (Levels II&ndash;V), group homes, and specialized placement settings. The Manager provides guidance, coaching, and operational support to staff to ensure timely placement decisions, placement stability, and consistent service delivery.
 
This position monitors placement operations, assists in resolving complex placement challenges, reviews operational and utilization data, and implements process improvements to support quality outcomes and efficient service delivery. The Manager establishes clear performance expectations, promotes accountability, and collaborates with the Director and internal partners to address operational needs and improve placement practices. Additionally, the Manager participates in the third-tier on-call rotation, providing operational support and guidance for after-hours and emergent placement needs.

Position Summary: This position is responsible for coordinating services, managing utilization, and authorizing initial and ongoing services for children and families involved in the child welfare system. The role ensures the effective, efficient, and fiscally responsible use of agency resources through oversight of service planning, authorization, delivery, reimbursement, and provider compliance.
Key responsibilities include fiscal management, providing support and consultation to Clinical Coordinators, and maximizing third-party reimbursement and alternative funding sources whenever available. The position participates in case review meetings, reviews and approves service requests, obtains and maintains all required documentation to support service authorizations and payments, prepares and processes requests for checks, reviews and authorizes invoices. The incumbent validates provider billing by reviewing submitted invoices and supporting documentation to ensure services billed, accurately documented, medically necessary, and compliant with agency standards, funding requirements, and contractual obligations. This includes reviewing provider progress notes to verify that they support billed services and meet established documentation standards.
This position is also responsible for tracking and monitoring Community Mental Health (CMH) funding, maintaining accurate funding and utilization data, and ensuring expenditures remain within authorized funding limits. The incumbent proactively monitors required documentation, authorizations, and funding-related deadlines; notifies team members of missing documentation, pending authorizations, and upcoming expirations; and collaborates with internal staff and external providers to ensure continuity of services and compliance with funding and regulatory requirements.
In addition, this position coordinates the onboarding of new service providers by obtaining required credentialing and contracting documentation, providing training on agency policies, utilization management procedures, documentation standards, billing requirements, and system workflows. The incumbent serves as a subject matter expert by providing ongoing technical assistance and system support to providers and agency staff, troubleshooting system issues, answering questions related to service authorizations, billing processes, and documentation requirements, and promoting consistent application of agency procedures and best practices. Also includes data management and reporting, to maintaining accurate utilization and authorization records within agency systems.
Additionally, this position promotes individualized, family-centered, culturally responsive, and trauma-informed treatment planning. The incumbent ensures that services and interventions are flexible, evidence-informed, and tailored to meet the unique strengths, needs, and cultural backgrounds of the children and families served. This role serves as a resource to staff and providers on utilization management, fiscal accountability, procurement requirements, funding guidelines, billing compliance, documentation standards, and electronic systems to ensure high-quality, compliant, and fiscally responsible service delivery.

4. 5.       Fiscal Management &ndash; Essential Function: This position is responsible for managing documentation required for specific use of specific dollars assigned requiring strong organization and fiscal management skills which require oversight of expenditures, funding utilization, and responsible stewardship of agency resources.
6.     Training- Essential Function: This position will assist with system training for both providers and case management to include training components to which involves ongoing technical assistance, consultation, and support to contracted providers.

POSITION OBJECTIVE:   Responsibilities include data management, data entry, data reporting and data validation.  In addition, this position obtains, maintains and processes client information to determine eligibility for Federal/State funding and Medicaid. In this role you will assist in resolving problems, locating missing/additional information and be responsible for creating and maintaining documentation which meets all Federal and State requirements for client eligibility.
 
ESSENTIAL FUNCTIONS: 

Accurately enters required program information adhering to strict deadlines
Obtains and maintains appropriate eligibility documentation 
Completes applications for Medicaid and Federal/State Funds
Maintains open communication with Protective Investigators and Case Managers responsible for supervising cases of children in care, obtaining information on income, household composition and family resources in order to determine eligibility
Provides technical assistance on the completion of all eligibility forms
Service Center Medicaid support: verification, provider changes and validation activities
Document tracking, checklist preparation and requests for more information
Queries systems to access specific information for distributing weekly/monthly reports to case management and senior management staff 
Performs routine data validation sampling 


Attends regular IES team meetings, staff meetings, service center meetings, case management meetings, in service trainings and conferences as needed. 
Assists with Medicaid plan enrollment activities and tracking.
Participates and documents CTS staffings.
Perform IES training activities such as Argos training, DCM orientation and ongoing FSFN support.
Participates in peer file review activities.
Participates in Federal Monitoring Reviews. 
Maintain frequent communication with internal staff regarding changes to case data and eligibility including but not limited to Records Room, Adoption, GAP, Master Trust and Operations staff.
Utilize established checklists, forms and task tracking tools identified in the Data Center Handbook to report work status and maintain required documentation. 
Participates in the agency strategic plan & quality improvement processes
Ability to safely & successfully perform essential job functions consistent with the ADA, FMLA, & other federal, state, & local standards, including meeting qualitative & quantitative productivity standards.

Position Summary: The Licensing Specialist serves as the primary point of contact for prospective foster parents and is responsible for the timely and thorough completion of foster home studies in accordance with Florida Administrative Code. This role evaluates applications and provides recommendations for the approval or denial of licenses. The specialist also assesses incident reports and licensing concerns related to potential foster homes, offering recommendations for appropriate resolution. Under the guidance of the Licensing Supervisor, the Licensing Specialist is responsible for collecting, organizing, monitoring, and distributing documentation to ensure licensing standards are met. This position supports the successful progression through the various levels of initial or re-licensure for foster care applicants, ensuring compliance with all regulatory requirements.
 
Technical/Functional Expectations
1.       Complete Licensing Home Studies-Essential Function: The position completes initial and/or re-licensure home studies inclusive of the assessment and evaluation of all household members and babysitters, to include background screening and abuse report history. 
2.       Background Checks &ndash; Essential Function: Review and evaluate criminal history reports, fingerprint results, child abuse and neglect registry checks, sex offender registry searches, and other required clearances. Ensures continuous compliance with Title IV-E requirements by monitoring and tracking background check renewals, preventing lapses in required screenings and clearances for foster parents and household members. 
3.       Home Visits to Family Foster Homes-Essential Function: This position will provide regular and ongoing home visits to family foster homes throughout the licensure process.
4.       Conduct Ongoing Assessments-Essential Function: This position will conduct ongoing assessments to determine capacity as indicated based on family skills, training, household composition, physical space and other determining factors.
5.       Liaison between Foster Parents and Case Managers &ndash; Essential Function: This position will serve as a Liaison between foster parents and case managers ensuring open communication and timely resolution of any identified concerns when applicable.
6.       Foster Care Referrals-Essential Function: This position will comply with Administrative Code requirements for any foster care referral on a licensed foster home. The licensing specialist will make recommendations for resolution of
all incident reports, foster care referrals and licensing concerns related to existing foster homes. This includes any joint visits for DCF institutional reports on assigned foster homes.

ESSENTIAL FUNCTIONS:

Utilize multiple tools, including but not limited to FSFN, FLEMMIS, CLEAR, and CCIS, to locate guardians on closed cases identified by the courts.
Request an update on the child&rsquo;s educational and medical status and determine if services are needed for the child.  If services are needed, community referrals for the child should be completed.  
Provide written reports and updates to the courts regarding status of cases, including the well-being of the child(ren).
If a child cannot be located or is not with the court approved guardian, the Coordinator is responsible for attending the scheduled court hearing to provide updates on efforts to locate the families. 
If a case re-opens, the Coordinator would staff the case with the assigned case management team.
Act as a liaison between guardians in need of intensive services and diversion and prevention programs.
Maintain monthly data regarding workload and other outcomes and output measures. 
Interpret collected data for the purpose of resource allocation, identification of gaps in services, tracking of service array usage and performance of service providers.
Assist with diversion related continuous quality assurance and improvement activities
Attend and participate in relevant training and meetings.
Participates in the agency strategic plan & quality improvement processes, as needed.

ESSENTIAL FUNCTIONS:





&middot;       Provide specialized assistance to Child Welfare teams and the children and families in care.
&middot;       Coordinate planning strategies and maintain positive relationships with agencies and         community members as applicable.

&middot;       Assist in the assessment of client&rsquo;s behaviors and emotional needs to determine the best possible community services to be implemented with/ for youth/ family.
&middot;       Act as a liaison with DCF/case management and the community

&middot;       Complete any necessary file documentation and process data through FSFN and other required data systems.

&middot;       Expected to be a part of the CPI initial consultations and 2nd  tier staffing&rsquo;s. Also available to be included in impromptu staffing regarding present and impending danger plans. Be a critical part in the decision of what services are available and can be implemented to ensure a child/ children is able to be kept in their home safely.
&middot;       Attach themselves to a CPI case that has been identified as needing prevention services and assist CPI with gathering information from parents/ other providers that the family may have been involved with or is currently with to move case forward and keeping the children safe without &ldquo;ongoing&rdquo; case management.
&middot;       Act as the conduit between providers and families. They are the linkage of services and ensure that contact between the provider and family has occurred and they are engaged. Also obtain all monthly documentation from providers and scan into the FSFN file cabinet and or update notes in FSFN timely and continuously.
&middot;       Ensure that parents sign the FSS Acknowledgement forms upon initial contact.
&middot;       Completes funding authorization form/ Diversion Supervisor signs the funding authorization and Diversion Manager approves and signs off for final approval.
&middot;       Screens all referrals to community providers and programs in the community.
&middot;       Maintain and complete ongoing Diversion tracking logs (weekly and as needed) 
&middot;       Participates in the agency strategic plan & quality improvement processes.

This position will be based at the Rockledge care center. The Family Allies Family Support Worker position serves as a paraprofessional support to Dependency Care Managers, with primary focus on coordinating and supervising Family Time with families involved with the Dependency Child Welfare System of Care. Family Support Workers will be responsible for maintaining records, program referrals, accompanying/transporting children and parents to therapeutic services, other providers, and supervising Family Time. The Family Support Worker will develop and maintain client records and program documentation based on first hand observations and assessments.

Technical/Functional Expectations:

Complete required training - Essential Function &ndash; Comply with training requirements, to include completion of required in-service training hours, to include Wrap-Around Training.
Safety - Essential Function &ndash; Facilitate and provide Supervision for families during Family Time activities.  Must act decisively, when necessary, to protect clients and model appropriate boundaries for parents and children during supervised Family Time.
Permanency - Essential Function&ndash; Adhere to and monitor Family Time Guidelines to successfully achieve permanency for children placed in out-of-home care.

Engagement - Essential Function &ndash; Adhere to the local System of Care values, goals and priorities for Brevard&rsquo;s children, youth and families; Utilize a family centered strength based approach to care; Monitor quality of care by ensuring appropriate visitation plans are in place to correlate to the needs of the family.
Support - Essential Function - Provide support to staff, children, caregivers and family members to ensure timely achievement of permanency; Coordinate cases with team members, providers, DCF and attorneys as needed.
Court Appearance and Preparation - Essential Function- At times this position may require a court appearance.  Case information must be reviewed prior to every hearing in order to be prepared to answer any questions from the Court regarding the status of the case, services, and progress towards permanency; Dress appropriately for Court appearances and ensure all court hearings are attended timely.

The Diversion Care Coordinator will coordinate the delivery and care coordination of services, ensure child safety, make referrals, provide community linkages and facilitate Family Team Conferences in a flexible, responsive, culturally competent service delivery system. This position will provide family preservation and stabilization support to children and families and coordinate with all members of the Family Team including caregivers, providers, DCF and informal and natural supports. This position will provide on call support to families served.
 
Technical/Functional Expectations:
 
1.       Receives referrals from Child Protective Investigators: This position will receive referrals from and interface with the Department of Children and Families Child Protective Investigators for families when a child has or children have been determined to be unsafe due to impending danger but the danger threat can be managed with a safety plan.
2.       Complete Family Assessment and Develop Case Plan: This position will be responsible for completing home visits every thirty days (minimally), will complete a family assessment and Strength and Cultural Discovery in order to develop an individualized care plan outlining family needs, goals and services.
3.       Service Utilization UM/Wrap: This is strongly recommended in order to manage and track the daily service utilization log, monitors costs and tracks 3rd party resources. Coordinates with providers to ensure service delivery in timely manner and to ensure needs are being addressed as detailed in the individualized care plans with family members and informal and natural supports.
4.       Authorization Coordination: This is strongly recommended to ensure timely delivery of appropriate services while managing the budget allocations and identifying alternative community resources as well as other funding streams in order to maximize resources and adhere to policies and procedures. Responsible for utilization and monitoring flexible funding and service coordination in order to meet family needs.
5.       Facilitate FTC's: This position will use family strengthening techniques to effectively bring a diverse team of professionals, family and community resources together in order to develop strategies, delegate tasks and identify ways to meet family needs. Ability to use judgement and a solution focused style is required to address differing points of view to achieve consensus within the FTC process while reframing family deficits. It is essential that the family voice drives the process and that decisions and actions have support of the group. 
6.       Family Preservation and Crisis Intervention: Provide family stabilization crisis intervention linkages, referrals through consultation to ensure family support and authorization of services. Conduct emergency FTC's when indicated and subsequent utilization reviews for continued authorization. Ensure development of interim crisis and safety plan as needed.

Permanency - essential function: Identify and develop plan to successfully achieve permanency for children placed in out-of-home care.
Service Delivery - essential function: Assess child and family needs; develop service plans; coordinate the delivery of services; ensure all referrals and linkages to children/parents/caregivers are completed.
Reports - essential function: Prepare all required court reports and submit within the required time frames; make recommendations; testify at court hearings regarding reunification, permanent guardianship, termination of parental rights, or other case management activities; ensure court orders are completed within required time frames.
Documentation - essential function: Develop and maintain case and program documentation according to contract; ensure documentation occurs within the SACWIS system and other related systems within required time frames.
Engagement - essential function: Adherence to the local SOC values, goals and priorities for Brevard&rsquo;s children, youth and families; Utilize a family centered strength based approach to care; Monitor quality of care by conducting face-to-face home visits with children minimally every 30 days, or with increased frequency as determined by case situations and supervisory direction; facilitate, monitor, and assess visitation between children, parents, and siblings to ensure safety and observe interactions.
Support - essential function: Provide support to children, caregivers and family members to assure stable placement and timely achievement of permanency; Coordinate cases with supervisor, team members, providers, DCF and attorneys as needed.
Court Appearance and Preparation - essential function: Review case information prior to every hearing in order to be prepared to answer any questions from the Court regarding the status of the case, services, and progress towards permanency; Dress appropriately for Court appearances and ensure all court hearings are attended timely.
Initiatives - essential function: Embraces and participates in initiatives and programs designed to enhance the System of Care.
On Call - essential function: This position is required to participate in the on-call function on nights and weekends.
Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting and training others, and performing duties of higher rated positions from time to time for developmental purposes.

Business Experience:

Valid Driver's License - Essential: A valid driver's license is required in order to attend meetings and other activities throughout the County and State.
Strong written and verbal communication skills: Strong communication skills are essential in order to clearly and concisely articulate programs, proposals, write policies and respond to program inquiries in a professional manner.
Collaborative, solution-focused approach: A collaborative, solution-focused approach is essential in order to foster and develop relationships and to build consensus among parties with conflicting positions and opinions.
Computer literacy: Intermediate skill in Word, Excel, SACWIS system are required.

Educational and Experience Requirements:

Bachelor&rsquo;s degree in social work or related area of study from an accredited college or university required. (Related degrees are Criminology, Psychology, Sociology, Counselling, Special Education, Education, Human Development, Child Development, Family Development, Marriage and Family Therapy)
Master's degree in Social Work or related field of study preferred
Two years&rsquo; experience working with at risk children and families preferred
