Thompson's PRTF is seeking a Case Manager who will assist with the successful discharge planning of our children as well as linking them to the necessary services and supports.
This position will be a part of a large, multidisciplinary team that provides treatment to North Carolina's most at-risk and traumatized populations. The Case Manager is responsible for ensuring that our children and families are linked with all the resources they need to achieve their goals.
The Case Manager role works directly with children and families as well as key stakeholders and external partners. Excellent docuemntation and organization skills are a must.
