We are recruiting for a licensed pharmacist to join our exceptional clinical care team.

Primary Functions:

  1. Uses available resources to gather information on drugs, equipment, and patient monitoring.
  2. Assists in the maintenance and cleaning of pharmacy work areas, and the completion of quality control and performance improvement procedures as requested by the Pharmacist In Charge.
  3. Acts as a resource and consultant to physicians, referral sources, and consumers regarding potential adverse reactions, and interactions, related to drugs dispensed, proper use of medications, administration supplies, and administration equipment.
  4. As a clinical expert, the clinical pharmacist stays knowledgeable of current clinical issues, industry practice standards, safety concerns, and accreditation standards and incorporates them into daily practice.
  5. Maintains appropriate documentation of the care delivered to consumers and of drug prescriptions and compounding as described in the Pharmacy Policy and Procedure Manual and as mandated by individual Oregon Board of Pharmacy requirements.
  6. Assists with employee orientation and preceptorship as needed.
  7. Completes assigned paperwork/document as directed in a timely manner.
  8. Conducts clinical quality checks such as PV1 and PV2, for prescriptions.
  9. Calls physician’s offices for clarification on any orders or prescriptions written by the physician, provides clinical advice regarding medication treatments, and obtains any and all information to ensure efficient, accurate, and safe dispensing of medications to the patient.
  10. Engages patients in detailed clinical counseling on all aspects of their medication regimens, disease states and issues or concerns that arise.
  11. Reports adverse events to the appropriate collector, such as MedWatch and Celgene, in a timely manner.
  12. Obtains insurance authorizations when Order Entry is unable, for example, peer to peer review.
  13. Directs activities of Call Center, Order Entry, and team members that relate to the Clinical Help Desk procedures.
  14. Establishes and maintains effective professional relationships with prescribers.
  15. Adheres to all state, federal, and local laws pertaining to drug dispensing and patient information, as well as company policies and procedures.
  16. Completes quality control checks on prescriptions to ensure all policies, procedures, and laws are followed.
  17. Takes prescription orders over the phone from physicians and patients.
  18. Provides patient specific consultations such as clinical assessments or rescue program consults.
  19. Answers the Clinical Help Desk phone as soon as possible.
  20. Utilizes the phone software program responsibly to ensure optimal customer service.
  21. Maintains appropriate records and computer entry of prescriptions and patient information.
  22. Documents patient interventions with adequate detail in both clinical and non-clinical records as appropriate.
  23. Investigates and solves insurance company denials if needed.
  24. Manages operational and personnel issues when the Manager or Pharmacist In Charge is unavailable.
  25. Conducts clinical assignments as needed.
  26. Assists in the implementation of new managed care prescriptions and other projects as needed.
  27. Possesses specialized skills to accept, check, and answer questions concerning prescriptions.
  28. Possesses communication skills to work effectively with all internal and external customers.
  29. Works well with various personalities and able to work as part of a team.
  30. Remains dependable and maintains appropriate levels of confidentiality.
  31. Possesses the ability to recognize needs of and express concern for people.
  32. Actively participates and provides support as needed to fellow leadership and quality program staff to ensure activities related to the company’s quality management program and quality improvement activities maintain priority focus and are effectively executed. This may include but is not limited to activities that promote objective and systematic measurement, monitoring, and evaluation of services and defined Quality Improvement Projects (QIPs) that may result from such efforts.
  33. Complies with the company’s Human Resources policies, Employee Handbook, Code of Conduct, and all department policies and procedures, including protecting confidential company, employee, and customer information; attending work punctually and regularly; and adhering to good safety practices in all activities.
  34. Performs other duties and responsibilities as assigned.
