Position Summary: The Family Allies Independent Living Care Manager Supervisor is responsible for the direct supervision and coordination of quality services and the young adults case management team focused on the provision of case management services for children ages 13-17 and young adults enrolled in Extended foster care.  This position leads a team who focuses on Independent Living preparation, their family members, caregivers, CLS attorneys, providers, and other appropriate professionals.  The Independent Living Care Manager Supervisor will become proficient in identifying and assessing the youth and young adults needs and requirements to ensure all Independent Living preparation requirements are met and maintain enrollment in Extended Foster Care.  The Independent Living Care Manager Supervisor adheres to the BFP System of Care principles of practice and provides family centered strength-based assessment, planning, linkage, coordination, and advocacy for children and families. This position works as a team member with key partners including parents, family members, foster parents, caregivers, guardian ad litem, DCF Protective Investigations, CLS attorneys, therapists, providers, and other appropriate professionals. The Care Manager Supervisor must have a current, active Child Welfare Case Management Certification from the Florida Certification Board.

Technical/Functional Expectations:

  1. Complete required certification – Essential Function: Maintain own and staff’s Child Protection Certification through completion of required in-service training hours and appropriate supervision activities.
  2. Safety Essential Function: Conduct, Develop, Monitor and Manage family functioning assessments, child safety assessments and required safety plans.
  3. Permanency - Essential Function: Identify and develop plan to successfully achieve permanency for children placed in out-of-home care.
  4. Service Delivery - Essential Function: Assess child and family needs; develop and monitor service plans; coordinate the delivery of services; ensure all referrals and linkages to children/parents/caregivers are completed.
  5. Reports Essential Function: Monitor/Prepare all required court reports and submit within the required timeframes; make recommendations; testify at court hearings regarding reunification, permanent guardianship, termination of parental rights, or other case management activities; ensure court orders are completed within required timeframes.
  6. Documentation Essential Function: Develop and maintain, case and program documentation according to contract; ensure documentation occurs within the SACWIS system and other related systems within required timeframes. Oversee professional, timely program case documentation and reporting, including the tracking and monitoring of client progress and outcomes.
  7. Engagement - Essential Function: Adherence to the local SOC values, goals and priorities for Brevard’s children, youth and families; Utilize a family centered strength based approach to care; Monitor quality of care by ensuring face-to-face home visits with children are completed minimally every 30 days, or with increased frequency as determined by case situations and supervisory direction; facilitate, monitor, and assess visitation between children, parents, and siblings to ensure safety and observe interactions.
  8. Support - Essential Function: Provide support to staff, children, caregivers and family members to assure stable placement and timely achievement of permanency; Coordinate cases with team members, providers, DCF and attorneys as needed.
  9. Court Appearance and Preparation - Essential Function: Ensure staff have reviewed case information prior to every hearing in order to be prepared to answer any questions from the Court regarding the status of the case, services, and progress towards permanency; Dress appropriately for Court appearances and ensure all court hearings are attended timely by self or assigned staff.
  10. Initiatives - Essential Function: Embraces and participates in initiatives and programs designed to enhance the System of Care.
  11. On Call - Essential Function: Participate in unit on call rotation for dependency case management.
  12. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes.

  8. Coaching/Mentoring- Coach/Mentor DCMs in the technical/functional expectations listed above and provide high-quality, effective services in individual treatment plan development, behavioral assessment and other direct treatment support, based on COA Standard TS 3.
  9. Positive Approach - Possess a positive attitude concerning all aspects of working in a challenging environment including significant patience and respect for peers and team members, as well as the children and families receiving program services

Business Experience:

  1. Valid Driver's License - Essential: A valid driver's license is required in order to attend meetings and other activities throughout the County and State.
  2. Strong written and verbal communication skills: Strong communication skills are essential in order to clearly and concisely articulate programs, proposals, write policies and respond to program inquiries in a professional manner.
  3. Collaborative, solution-focused approach: A collaborative, solution-focused approach is essential in order to foster and develop relationships and to build consensus among parties with conflicting positions and opinions.
  4. Computer literacy: Intermediate skill in Microsoft Suites and SACWIS system are required.

Educational and Experience Requirements:

  1. Bachelor’s degree in social work or related area of study from an accredited college or university required. (Related degrees are Criminology, Psychology, Sociology, Counselling, Special Education, Education, Human Development, Child Development, Family Development, Marriage and Family Therapy)
  2. Child Welfare Certification, issued from the Florida Certification Board and at least 2 years’ experience working with at risk children and families preferred.
  3. Strong knowledge about dependency case management and child welfare system of care.
