Position Summary: TheBrevard C.A.R.E.S. Family Support Services Supervisor manages a team of Care Coordinators providing intensive care coordination of families in the Family Support Services Program. The Brevard C.A.R.E.S. Family Support Services Supervisor will provide coaching to the C.A.R.E.S. Care Coordinators in the Wraparound learning process; educating the staff on every phase of the process and providing support and feedback to ensure that staff are implementing each phase with high-fidelity. This process occurs through a series of documentation reviews, role-play activities and on-site observations in the family home. Additionally, the Brevard C.A.R.E.S. Model requires that data is collected to ensure that the process is outcome driven. This attention to both process and outcome ensures fidelity to the model. This also included supervisory review of case records and compliance with Florida Statute Chapter 39, Florida Administrative Code, and all agency policies, operating procedures and applicable COA standards.   

Technical/Functional Expectations:

  1. Management/Leadership of Family Support Services - Essential Function: This position will have direct reports of Care Coordinators and interface with the Department of Children and Families Child Protective Investigators for those children and families identified to be served through Family Support Services.  This position will also complete Performance Evaluations and appraise the employee’s job results and performance in order to develop, counsel or discipline direct reports.
  2. Wraparound subject matter expert - Essential Function: Serve as Wraparound subject matter expert to lead, coach and develop staff to excellence in this model.
  3. Leadership in developing partnerships-Essential Function: This position will co facilitate Family Team Conference and assist with complex cases when indicated. This position will promote and provide leadership of strong relationships and consensus building with family members, family partners, providers and the community.
  4. Conduct Utilization Reviews - Essential Function: This position will conduct utilization reviews of case records, ensure consumer satisfaction, and use of wraparound fidelity tool is incorporated into every meeting. This includes the ongoing review of care plans to ensure they have clearly defined outcomes.
  5. Care Planning Resources – Essential Function: Access 3rd party, Medicaid and community resources and ensures inclusion in care planning in order to maximize resources.
  6. Crisis Interventions – Essential Function: Provide crisis intervention linkages, referrals through consultation to ensure family support and stabilization in order to follow-up with emergency FTC for continued authorization. Ensures interim safety plan and follow up development in order to protect clients and agency's best interests.
  7. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting and training others, and performing duties of higher rated positions from time to time for developmental purposes.

Business Experience:

  1. Valid Driver's License & Appropriate Insurance - Essential: This position also requires a valid driver’s license in order to travel throughout the County and requires $100,000/$300,000 bodily injury insurance coverage (per FL Statute) in order to transport clients.
  2. Computer skills (Microsoft and Web based application): This position uses Microsoft and a web-based application for daily entry of case activity and the utilization management of service authorizations.
  3. 2+ years of Team Facilitation Skills with At Risk Families: This position works collaboratively with both informal and formal professional supports and must demonstrate the ability to effectively partner with collaterals and service providers.
  4. 1+ years’ Experience as a Supervisor/Manager: This position requires at least 1 year of experience as a Supervisor/Manager of family-based programs.

Educational and Experience Requirements:

  1. Bachelor’s Degree required.
  2. Master’s Degree in Human Services Related field preferred.
  3. 2 to 5 years working within a Wraparound Model of Care Coordination required.
  4. 5 to 8 years related experience preferred.
