Starting Annual Salary: $40,287

Position Summary: This position oversees and manages the authorization and utilization of services delivered for at risk families who have been referred for Care Coordination through the Mobile Response Team.  The Mobile Response Team operates 365 days a year 24/7.   This position maintains information regarding the environment of current services to ensure a flexible, responsive, culturally competent service delivery system.  This position is funded by a federal grant.  Although ongoing funding is expected, employment could end because of the loss funding.

Technical/Functional Expectations:

  1. Authorization Coordination-Essential Function: This is strongly recommended to ensure timely delivery of appropriate services while managing the budget allocations and identifying alternative community resources as well as other funding streams to maximize resources and adhere to policies and procedures. Responsible for utilization and monitoring flexible funding and service coordination to meet client needs.
  2. Care Planning Resources: Access 3rd party, Medicaid and community resources and ensures inclusion in care planning to maximize resources.
  3. Crisis Interventions: Provide crisis intervention linkages, referrals through consultation to ensure family support and stabilization to follow-up with emergency FTC for continued authorization. Ensures interim safety plan and follow up development to protect clients and agency's best interests.
  4. Facilitate FTC's: This is strongly recommended to effectively bring a diverse team of professionals, family, and community resources together to provide support to the family to develop strategies and identify needs to address their issues. Ability to use judgement and a solution focused style to address differing points of view to achieve consensus within the FTC process. Ensure that the family voice drives the process, and that decisions and actions have support of the group.
  5. Leadership of Trauma Informed Care Initiative: This position provides leadership of the Trauma Informed Care initiative to improve outcomes for youth in the Brevard County System of Care.
  6. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes.

Business Experience:

  1. Valid Driver's License & Appropriate Insurance - Essential: This position also requires a valid driver’s license in order to travel throughout the County and requires $100,000/$300,000 bodily injury insurance coverage (per FL Statute) in order to transport clients.
  2. Computer skills (Microsoft and Web based application): This position uses Microsoft and a web-based application for daily entry of case activity and the utilization management of service authorizations.
  3. 2+ years of Team Facilitation Skills with At Risk Families: This position works collaboratively with both informal and formal professional supports and must demonstrate the ability to effectively partner with collaterals and service providers.

Educational and Experience Requirements:

  1. Bachelor’s Degree required.
  2. Master’s Degree in Human Services Related field preferred.
  3. 2 to 5 years related experience required.
  4. 5 to 8 years related experience preferred.
