Summary of Qualifications:
The Clinical Supervisor, HRA is responsible for coordinating, monitoring, evaluating and facilitating high-quality, cost-effective Health Plan Health Risk Assessment completion. Assists in the development, organization, direction and maintenance of HRA functions for the department. The supervisor provides oversight in the day-to-day HRA activities and works independently with little supervision.

Responsibilities:
•Coordinates Health Risk Assessment services for members as prescribed by the Health Plan’s policy, CMS/State guidelines, and
member’s eligibility.
•Monitors staff productivity and implements the performance management process, as appropriate.
•Monitors timely HRA completion activities by reviewing daily and weekly staff productivity reports/dashboards, adjusting staffing
and work assignments as needed to maintain productivity standards.
- Conducts telephonic and face to face assessments as requested.
•Assists in implementation of operational changes in order to improve efficiency of processes and meet the Health Plan’s strategic
goals.
•Serves as a clinical resource to the HRA Assessors.
•Resolve complaints in a positive manner with tact and diplomacy within established time frames.
•Continuously monitors contract compliance of Health Risk Assessment services and adjusts work plan, if needed.
•Maintains awareness of state and federal legislation effecting HRA services.
•Ensures HRA staff complies with Health Plan, State, and CMS policies and procedures.
•Assists with orientation and training of new HRA Assessors.
•Counsels HRA Assessor staff, when needed.
•Functions as a facilitator with regard to workplace departmental issues.
•Promotes Health Plan’s overall organizational and financial goals as well as improvement in overall quality of the member’s health
care as the primary goal of any initiative.
•Responsible for the daily management of appointed staff and work in collaboration with other departments to achieve contractual
outcome goals.
•Participate in regular team meetings for communication and feedback, staff training and development, sharing of program
results, and collaborative problem solving.
•Complies with all guidelines established by the Health Plan, the State, the Centers for Medicare and Medicaid (CMS) and guidelines
set forth by other regulatory agencies, where applicable, and defined in the Universal American Corporate and department
policies.
•And all other duties assigned by the manager and/or supervisor.

Required Experience
•Social Work or Nursing licensure required
•Bachelor’s degree in nursing, social work, health and gerontology or a related field required
•Minimum of one year relevant supervisory experience in a hospital, health care or human services setting
•Experience in conducting social and health assessments is preferred
•Personal computer experience should include working with Microsoft Word, Excel, PowerPoint and Outlook at the intermediate

level at a minimum
•Experience identifying and working with various community resources and services
