Due to our exceptional growth and success, we are recruiting for a Nutritional Care Coordinator I to join our team in Nashville, TN!
Position Summary:
The Nutritional Client Care Coordinator is responsible for coordinating enteral patient care in an organized manner, and they work with the Registered Dietitian to provide clinical follow up as issues are identified.
Roles & Responsibilities:- Facilitate the effective operational work flow of new and ongoing enteral patients.
- Follow up with enteral patients at least monthly for patient clinical update, including tolerance of tube feeding, and review supply needs.
- Communicate any patient issues that are not within boundaries, to registered dietitian.
- Refill prescriptions in CPR, create delivery ticket for formula and supplies, and schedule delivery/shipment for patient.
- Troubleshoot enteral tube feeding pump problems as needed, and schedule delivery of new pump if necessary.
- Ensure patient information (patient address, height/weight, doctor, insurance, etc.) is updated, and accurately documented in CPR.
- Assist dietitians as needed, with entering orders on new referrals into CPR.
- Update enteral orders every 90 days or when there are changes in therapy, complete discharge documentation, and discharge enteral patients from CPR.
- Communicate with HomeChoice Partners branches regarding patient discharge and pick-up of equipment.
- Other duties as assigned.
Education, Training & Experience:- High School graduate
- Experience as a diet technician, LPN, or medical assistant is preferred, but not required.
