SUMMARY
 
Coordinates and performs total nursing care for patients in the home setting; administers skilled nursing care; provides teaching for patients and caregivers in home medical management; provides leadership by working cooperatively to establish priorities and goals of patient care.
 
ESSENTIAL FUNCTIONS
To perform this job successfully, an individual must be able to perform each essential function satisfactorily, with or without reasonable accommodation.
 
1.     Provides infusion and other skilled nursing care to patients including: patient assessment and reassessment, patient and caregiver teaching, development and revision of the Plan of Care/ Plan of Treatment, implementation of physician orders, monitoring patient response to therapy, response to patients needs in an emergency,   psychological support and ongoing case management.
2.     Provides professional nursing care for the safety and well being of home infusion patients.
3.     Collaborates with pharmacy staff to develop and monitor goals of patient therapy through the infusion pathway.
4.     Communicates with physician and other members of the health care team as appropriate to convey information regarding patient status and to make recommendations.
5.     Coordinates patient referrals and admission with other members of the health care team in order to meet home infusion and other patient care needs as assessed.
6.     Obtains correct, appropriate and timely prescriptions and specific care instructions from referring physician, including prescriptions for all nursing orders.
7.     Identifies learning needs of patient and/or caregiver.
8.     Provides appropriate intervention and instruction for patient and/or caregiver to safely administer care at home.
9.     Communicates patient learning needs and findings to health care team members, including pharmacy, nurse coordinator, customer service staff and physician/referral source, as appropriate.
10.   Appropriately documents patient care activity, home infusion nursing services provided and all communications with patient and physician.
11.   Conducts ongoing chart review per policy.
12.   Forwards required documentation to branch, physician, payors and referral sources within policy or contractually stated time lines.
13.   On an on-going basis, monitors patient progress to collaborative home infusion goals to assess medical, psychosocial and home care environment.
14.   Ensures patient competence and compliance with all self-care, including procedures and infection control; re-evaluates as necessary and documents/communicates the need for re-education.
15.   Maintains timely communication with physician regarding patient status and procedures.
16.   Provides nursing care and services in accordance with company policies and procedures and all applicable accrediting bodies, federal, state and local regulations.
17.   May assume leadership role for the department in the absence of a Manager.
18.   Performs other duties as assigned.

  • The employee is frequently required to stand, walk, sit, reach outward, and handle/finger.
  • The employee is occasionally required to reach above shoulder, climb, crawl, squat, kneel and bend.
  • The employee must be able to frequently push/pull 15lbs or less and occasionally push/pull up to 100lbs.
  • The employee must be able to lift/carry up to 15lbs frequently and up to 100lbs occasionally.
  • Specific vision abilities required by this job include close vision, distance vision, peripheral vision, and the ability to adjust focus.
  • The employee must have sense of touch and the ability to wear Personal Protective Equipment (PPE), such as gloves, masks, gowns and/or scrubs.

  • Work is normally performed in patient homes and/or a typical interior/office work environment.
  • The noise level in the work environment is usually quiet to moderate.
 
EDUCATION, TRAINING AND EXPERIENCE REQUIREMENTS
 
Graduate of an accredited school of nursing required, BSN preferred. Current Registered Nurse licensure in good standing in state of practice required.  CPR certification is required. CRNI preferred.  A minimum of two (2) years current nursing experience in a clinical specialty involving infusion therapy required, including patient assessment skills, venous access skills and emergency patient care, with at least one (1) year of home health experience required.  Excellent interpersonal, verbal & written communication skills and organizational skills required. Must be able to assume a flexible work schedule and have the ability to travel 50-80% as needed. Must have valid driver’s license as well as auto insurance and maintain a driving record which meets company’s standards.

  • Proficient in MS Office.
 
 
Language Skills
Ability to read, analyze and interpret clinical information appropriate to duties and responsibilities assigned.  Ability to operate information systems, follow policy, complete required forms, and prepare reports.  Ability to effectively present information and respond to questions from referral sources, managers, patients, employees, payors and the general public.
 
Mathematical Skills
Ability to perform basic mathematical calculations.
 
Reasoning Ability
Ability to define problems, collect data, establish facts, and draw valid conclusions.  Ability to use critical thinking to interpret an extensive variety of abstract variables.
