Description
1. Serves as a liaison to Veterans and their families, Community Nursing Homes (CNH), VA Medical Centers, and any other Federal, State, or Private agencies as warranted 2. Case manages Veterans in the CNH program. This includes monthly CNH visits with follow-up documentation in the Computerized Patient Record System, monitoring overall quality of care in the CNH's, assessment for continued need and level of CNH care services, making referrals for specialty outpatient services, supplies and equipment and arranging transportation 3. Provides counseling and health teaching to individuals or Community Nursing Homes. 4. May participate as a member of the Community Nursing Home Inspection Team, conducting initial, annual and focused “Quality of Care” visits 5. Participates in CNH program staff development, orientation processes and monthly meetings 6. Reports Quality Improvement data, sentinel events or adverse patient occurrences, and participates in the development of policies and procedures 7. Provides mentoring/instruction to students on the Community Nursing Home Program. 8. Provide assistance in CNH discharge planning as well as coordinating placement in a CNH 9. Responds to outside referrals such as community hospitals, community nursing homes, veterans and family members living at home and any other agencies, such as Hospice, etc. 10. The Community Health Nurse is responsible and accountable for making monthly and as-needed visits to monitor Veterans served by the Contract Community Nursing Homes, Contract Adult Day Health Care Centers and select FEE Basis arrangements
Required Skills

1. Citizen of the United States

2. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing, however, documentation from the Commission on Graduates of Foreign Nursing Schools (CGFNS) or International Consultants of Delaware (ICD) will be required to verify foreign education, along with transcripts

3. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

4. Must be proficient in spoken and written English.

5. Basic Life Support (BLS) certification from the American Heart Association, American Red Cross, or Military Training Network

6. Must have a valid Driver’s license to visit veterans in contracted Community Nursing Homes

7. Basic computer literacy proficiency with in-depth experience with the use of the use of Microsoft Office programs or comparable word processing, spreadsheet and/or graphic software and the ability to learn new programs specific to the VA

8. A Bachelor’s Degree in Nursing (BSN) preferred

9. At least 3 years of successful registered nursing practice

10. Experience in Community Health Nursing preferred

PHYSICAL REQUIREMENTS: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, kneeling, pulling, and pushing. Transferring patients and objects may be required. The candidate may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The candidate may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The candidate will be working closely with others and may have protracted or irregular hours of work.








Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research. Education/Experience Requirements:

· Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.

· Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.

· Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.

· Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.

· Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
