As our Care Management Clinical Pharmacist you would act as a critical member of the patient care team providing high level  patient and physician communication, pharmacist verification of prescriptions, and appropriate  clinical interventions to ensure improved outcomes and patient experience.  The Clinical Pharmacist provides clinical pharmacy services through the appropriate evaluation of physician orders and safe and efficient dispensing of medications, supplies, and administration equipment. The pharmacist may supervise the activities of pharmacy support personnel, as well as participate in quality control and performance improvement procedures.

Primary Functions:

  1. Uses available resources to gather information on drugs, equipment, and patient monitoring.
  2. Assists in the maintenance and cleaning of pharmacy work areas, and the completion of quality control and performance improvement procedures as requested by the PIC.
  3. Acts as a resource and consultant to physicians, referral sources, and consumers regarding potential adverse reactions, and interactions, related to drugs dispensed.
  4. Acts as a resource and consultant to physicians, referral sources, and consumers regarding the proper use of medications, administration supplies, and administration equipment.
  5. Stays knowledgeable of current clinical issues, industry practice standards, and accreditation standards and incorporates them into daily practice.
  6. Maintains appropriate documentation of the care delivered to consumers and of drug prescriptions and compounding as described in the Pharmacy Policy and Procedure Manual and as mandated by individual State Board of Pharmacy requirements.
  7. Assists with employee orientation and preceptorship as needed.
  8. Completes assigned paperwork/document as directed in a timely manner.
  9. Conducts clinical quality checks such as PV1 and PV2, for prescriptions.
  10. Calls Physician’s offices for clarification on any orders or prescriptions written by the Physician, provides clinical advice regarding medication treatments, and obtains any and all information to ensure efficient, accurate, and safe dispensing of medications to the patient.
  11. Counsels patient on all aspects of their medication regimen.
  12. Reports adverse events to the appropriate collector such as, MedWatch and Celgene, in a timely manner.
  13. Obtains insurance authorizations when Order Entry is unable, for example, peer to peer review.
  14. Directs activities of Call Center, Order Entry, and team members that relate to the Clinical Help Desk procedures.
  15. Establishes and maintains effective professional relationships with prescribers.
  16. Enforces all state, federal, and local laws pertaining to drug dispensing and patient information, as well as company policies and procedures.
  17. Completes quality control check on prescriptions ensuring all policies, procedures, and laws are followed.
  18. Takes prescription orders over the phone from Physicians and patients.
  19. Provides patient specific consultations such as clinical assessments or rescue program consults.
  20. Answers the Clinical Help Desk phone as soon as possible.
  21. Utilizes the phone software program responsibly to ensure optimal customer service.
  22. Maintains appropriate records and computer entry of prescriptions and patient information.
  23. Documents patient interventions with adequate detail in both clinical and non-clinical records as appropriate.
  24. Investigates and solves insurance company denials if needed.
  25. Manages operational and personnel issues when the GM or PIC is unavailable.
  26. Conducts clinical assignments as needed.
  27. Assists in the implementation of new managed care prescriptions and other projects as needed.
  28. Possesses specialized skills to accept, check, and answer questions concerning prescriptions.
  29. Possesses communication skills to work effectively with all internal and external customers.
  30. Works well with various personalities and able to work as a team.
  31. Remains dependable and maintain levels of confidentiality.
  32. Possesses the ability to recognize needs of and express concern for people.
  33. Performs other duties and responsibilities as assigned.
