Job Description:

The Community Health Worker (CHW) will be an integral member of the CityLife Neighborhood team. They will work closely with the operations and clinical staff, including but not limited to: operations leads, providers, nurses, clinical assistants, and social workers.

Requirements:

  • Conduct patient outreach and engagement activities including face-to-face, mail, electronic and telephone contact, including the support of patient recruitment
  • Obtaining informed consent/assent
  • Support the implementation of community health initiatives
  • Promote adherence to medical treatment through identification of barriers and development of mutually agreed upon compliance strategies
  • Assist organization with navigating healthcare system and community support including arranging transportation and scheduling appointments
  • Identify available community-based resources and coordinate referrals and follow-up on compliance
  • Utilize site-specific technology link services and resources, and communicate among the team, providers and members
  • Collect and report data for the program
  • Provide patient care information to care team providers
  • Electronically document activities, patient information and interventions in designated tracking system
  • Provide in-home assessments and support as indicated
  • Other duties as assigned

Qualifications:

  • Exhibits natural leadership ability or currently involved as a community leader
  • Expresses interest in community betterment and outreach, will a willingness to learn and show flexibility
  • Direct experience or knowledge of the community to be served
  • Excellent oral communication skills
  • Ability to establish positive, supportive and trusting relationships with and among patients and colleagues, with the desire to work collaboratively and effectively within a team
  • Knowledge of available social services and resources through personal experience preferred
  • Proven ability to collect and document data and information
  • Experience with motivational interviewing
