Description

Position Description: This role provides support to patients in our urgent and primary care facilities while collaborating with a team of medical providers. The ideal candidate will be focused on the delivery of quality care in accordance with the company’s culture of outreach and support.  This candidate will be responsible for conducting patient visits related to acute and chronic care management.  They will assist with the development, implementation, management, and achievement of specific programs / site objectives and initiatives, including coding and partnership for quality. Of utmost importance, this role is for an individual comfortable in a startup environment where an ‘all hands on deck’ mentality is key in fostering a positive team feel in a time of great growth.

Some Essential Job Functions Include:

  • Conduct patient visits for urgent care and primary care CityLife facilities.
  • Assist team members with inbound clinical triage.
  • Manage acute care and chronic care activities.
  • Identify, develop, and implement clinical protocols and obtain approval from physicians for patient treatment.
  • Assist with patient education activities.
  • Conduct presentations and community sessions pertaining to disease specific process and wellness.
  • Monitor the quality of care delivered to our members.
  • Complete chart reviews on a regular basis and in a timely manner.
  • Provide and receive feedback on quality of care being delivered to ensure JCAHO and other regulatory standards are met.

    Qualifications:

  • Graduate of an accredited program for Nurse Practitioners / FNP
  • Advanced education and training in the diagnosis and management of common medical conditions and chronic illnesses
  • Current licensure as a Nurse Practitioner in state of [Pennsylvania/NJ]
  • Cardiac Life Support (ACLS PALS) Certification. Current Certification by the American Academy of Nurse Practitioners or American Nurse Credentialing Center, (NP)
  • Meets all requirements in order to obtain associate medical staff membership and required privileges of service
  • Eligible for a DEA and a NPI #
  • Experience with EH records
  • Experience in a family practice and urgent care environment within a team based-setting
