GERIATRIC CARE MANAGER (RN/LPN) – PART TIME - $1500.00 Sign-On Bonus!  

Position Summary

Performs direct client “Geriatric Care Management” services, working with clients and families to navigate the aging process, monitor and enhance quality of life, problem solve, educate and advocate on behalf of the client through a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges. Working with families, the expertise of care management professionals provides the answers at a time of uncertainty. This role will guide and lead families to the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time off work for family caregivers. 

Qualifications

  • Minimum 2-5 years of experience
  • RN or LPN (active license) - REQUIRED
  • Home Health, Long Term Care, and/or ICU experience preferred
  • Must be able to work 30 hours per week minimum
  • Must be able to be on-call for after-hours emergencies and 2 weekends per month

Responsibilities

Client Visits

  • Conduct assessments of client needs
  • Monitor medical needs and conditions; observe symptoms and relay to health care providers
  • Monitor medications
  • Educate client about resources
  • Accompany on doctor visits to doctor’s office or at residence
  • Ask client about satisfaction with caregivers and make adjustments as needed
  • Problem solve various issues presented by the client
  • Manage client expectations
  • Visit clients without family

Behind the Scenes

  • Schedule and coordinate medical appointments
  • Coordinate care with medical providers, facilities, and families
  • Purchase needed items (e.g., clothing)
  • Procure assistive devices and make sure they arrive and are in use
  • Assist client in appeals (academic, financial assistance, etc.)
  • Research living facilities for clients
  • Get bill-paying checks signed and mailed timely
  • Advocate for clients
  • Manage caregiver services
  • Coordinate and navigate process to obtain resources
  • Foster client self-determination and independence
  • Proactively follow up with numerous providers/vendors servicing the client such as doctors’ offices, hospitals, discharge workers, caregivers, pest control services, and, sometimes, families.
  • Attend care plan meetings at a facility (may include nurse, physical therapist, occupational therapist, speech therapist, dietician, social worker)
  • Follow-up with rehabilitative services
  • Arrange for caregivers to take client to appointments
  • Communicate with doctors regarding medications

Customer-Focused Activities

  • Act as a single point of contact for the client, the family (trust company), and care givers for these activities, among others:
    • Research options
    • Match client needs to services
    • Advocate for client
    • Support customer
    • Coordinate services
    • Schedule appointments
    • Follow-up to ensure plans are acted on
    • Problem resolution
  • Communicate with client’s family about resources and care
  • Report to customer by email or by phone as appropriate
  • Facilitate communication between providers and family or trust officer and providers
  • Assist customer to hire personal care agency and ensure client satisfaction
  • Gather documentation regarding previous personal care agency for customer (client’s family) to submit to insurance company for reimbursement
  • Be family’s eyes and ears on the ground
  • Manage family’s expectations

Supporting the Business Activities

  • Maintain timesheets
  • Maintain licenses and credentials
  • Update notes in the system relevant to completion of billing. All work must be documented thoroughly and in a timely manner.
  • Follow up incessantly with clients and proactively seeks opportunities to assist client versus waiting for the client to request services.
