Position Description:

Reporting directly to the Clinic Operations Leader, the Medical Records Specialist collects, maintains and distributes all member documents. Responsibilities include maintaining the electronic medical records (EMR) system for all members while managing document control to ensure that the care team has the most up to date documentation in a timely manner. The Medical Records Specialist also responds to requests and correspondence, assembles highly confidential and sensitive information for internal and external parties.

Essential Duties:

  • Manage inbound faxes and index all documents to members’ charts.
  • Extract data, such as lab values and test results into data fields in the members’ charts.
  • Request medical records to external providers, such as previous PCP charts, and index to member’s charts.
  • Send progress notes from Urgent Care visits to external PCPs in the community.
  • Provide faxing/document support to care teams, as needed.
  • Manage incoming and outgoing information received from specialists, hospitals, and emergency rooms.
  • Accurately update patient information in the EMR system (i.e. demographics, contact preferences, insurance information etc).
  • Maintain member privacy at all times by following HIPAA guidelines.
  • Understand and utilize EMR system properly.
  • Work in partnership and develops strong professional relationships with team members. 
  • Collaborate with entire primary care team to ensure member’s healthcare needs are being met.
  • Other duties as assigned by supervisor

Qualifications/Skills:

  • High school diploma or general education diploma (GED)
  • Associate’s Degree in business or a related field preferred
  • Medical Office Administration courses and experience in a medical office environment a plus
  • Must have excellent communication skills
