The social worker evaluate the patient's comprehensive health care needs in the home environment and implements an appropriate plan according to these needs utilizing the home care team, physician, community resources, and the patient's support system. The social worker utilizes critical thinking skills related to the home care of the patient and implements the physician ordered plan of care. The social worker provides social work procedures and assessments and assists patients and families in finding resources needed to care for the patient.
Qualifications
- 1 year experience as a social worker. Home Care experience preferred.
- Current Kentucky license as a certified social worker.
- Current Kentucky driver's license
- Current BCLS
- MSSW and licensed social worker required.
- Full Time, 40 hours a week
