Position Description: Reporting to the Medical Director, the Clinical Pharmacist collaborates with a multi-disciplinary team composed of physicians, nurse practitioners, nurses, behavior therapists and clinical assistants providing optimal patient care in a primary care setting.

Essential Duties:

  • Conducts comprehensive appraisals of patients' health by performing medication reconciliations
  • Evaluates drug therapy, safety and efficacy through direct patient involvement and review of electronic medical records
  • Develops, documents and executes therapeutic plans with collaborating physicians and nurse practitioners
  • Establishes collaborative practice agreements between physicians, nurse practitioners and clinical pharmacists 
  • Delivers chronic disease state management according to established collaborative practice agreements including, but not limited to diabetes, HTN, COPD, and warfarin management
  • Provides medication and disease state education, blood glucose monitor training, inhaler training, and insulin training to patients and patient caregivers
  • Administers medications and immunizations according to pre-established protocols
  • Assists patients in enrollment of patient assistance programs 
  • Coordinates medication and sample inventory management for assigned clinics
  • Researches and responds to drug information questions

Qualifications/Skills:

  • Ability to demonstrate reliability and responsiveness to members, families and team members

Position Requirements:

  • PharmD from an accredited institution
  • PGY1 residency or three (3) years’ experience in an ambulatory care setting
  • State of PA Pharmacist
  • CPR certification
  • Immunization certification

Preferred Requirements:

  • Must have experience with direct patient and prescriber contact, including administration of medication
  • Board certification preferred
  • Experience in disease state management and independent clinical work
