Salary: $49,700/year
Position Summary: The Care Coordinator is responsible for facilitating the Wraparound process for youth and young adults in or previously in foster care that are eligible to receive services and support through the youth services department. The Care Coordinator coordinates and schedules Family Team Conferences to promote permanency and prioritizes urgent requests. The Care Coordinator maintains information regarding the environment of current services to ensure a flexible, responsive, culturally competent service delivery system. This position is a 2-year Federal grant funded position.
Technical/Functional Expectations:
1. Family Team Conferences - Essential Function: Coordinate and facilitate Family Team Conferences in order to achieve consensus among family and other participants in developing an appropriate plan based on the needs of the families and to ensure compliance with FPOCF procedures, values, and standards.
2. Service and Treatment Delivery - Essential Function: Ensure services and treatment are delivered from a
strength based framework and customized in order to meet individualized needs of consumers and are
culturally competent.
3. Utilization Management--Essential Function: Authorization and monitoring of resources, including agency
and community, to meet client’s needs.
4. Relationship Maintenance--Essential Function: Foster and maintain ongoing relationships with family
members, care managers, providers, and identified resources in order to call on others to implement
client plans.
5. Adhere to SOC Values and Principles-Essential Function: This is required in order to embrace and promulgate the values and principles of the System of Care on a consistent basis with each family and
interaction.
6. Planning/Scheduling/Meeting multiple deadlines: Plan and schedule meetings, conferences, and travel schedules in order to maintain staff schedules.
7. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned,
assisting and training others, and performing duties of higher rated positions from time to time for
developmental purposes.

Business Experience:
1. Minimum of 2 years knowledge of child welfare systems: This is required in order to understand and successfully function within our system of care and to adhere to Florida statute and code.
2. Strong computer/data entry skills: This experience is required in order to maintain our utilization
management system.
3. 3+ years’ Experience working with families and children: This experience is essential in order to
effectively work with children and families within our system of care.
4. Collaboration, negotiation and problem-solving skills: This experience is essential in order to effectively
facilitate the Wraparound Family Team conferencing process.
5. Valid Driver's License & Appropriate Insurance - Essential: This position requires a valid driver’s license in
order to travel throughout the County and requires $100,000/$300,000 bodily injury insurance coverage
(per FL Statute) in order to transport clients.
Educational and Experience Requirements:
1. A high school diploma or GED equivalent (official transcripts required) AND four or more years of full-time social work or human services experience.
- An associate degree from an accredited college or 60+ college credits from an accredited college AND two years of full-time social work or human services experience or two years of professional work experience. Examples of work experience could be, but are not limited to:
- Guardian Ad Litem or similar child advocate role
- Family Support Worker
- Group Home Staff
- Teacher’s assistant/aide
- Childcare provider/staff
- Therapeutic Assistant
- Behavioral health technician
- Family intervention specialist
- Home health aide
- Nurse (LPN or RN) or similar profession
- Nursing facility assistance
- Emergency Medical Services (EMS
- Other professional jobs that require an assessment of factors that can contribute to trauma or protective capacities with children and families.
- Bachelor’s Degree, Human Services or Social Work preferred. (Related degrees are Criminology, Psychology, Sociology, Counselling, Special Education, Education, Human Development, Child Development, Family Development, Marriage and Family Therapy although other degrees may apply.)
- 1 to 2 years related experience required.
- 2 to 5 years Wraparound and related experience required.
- You must obtain a Child Welfare Case Manager Certification within 12 months of completion of paid pre-service training.
