Description At Source2 in support of a leading healthcare company, our mission, quite simply, is to improve the health of those we serve. Our client is managing a new Care Coordination program servicing high risk Medicaid members throughout the state of California. The goal of the program is to assess and develop a plan of care with the members, focusing on care coordination and outcomes based decision making.

Are you ready to make a difference and join this new and exciting program? We are looking for a seasoned RN to provide field-based engagement, clinical/social assessment, and interventions for health plan members. The RN Care Manager works closely from field, along with members of call center care management team, to assess the member’s needs of healthcare services by medically and socially complex plan members. The RN will work assess member’s in their homes, traveling within the designated area.
Responsibilities:
• Go to members home and complete a face to face assessment with the member or the member’s designee and to create an appropriate Plan of Care for that member based off of the assessment and document those findings.
 
 
Experience/Qualifications:
•Associates Degree in Nursing required, BSN/MSN preferred
• Valid and unrestricted California RN Licensure required
• Minimum 3 years hands-on nursing experience required. Work in clinical setting serving medically or socially complex patients preferred.
• Previous experience in care management, disease management and/or case management preferred. Behavioral/mental health experience helpful.
• Public health nursing or home health background preferred
• Knowledge of medically complex/fragile, elderly patients with complex socioeconomic issues
• Knowledge of adult learning and behavior change principles preferred
• Knowledge of community and social resources (transportation, housing and food)
• Clinical and social assessment skills, consistent with state licensure scope of practice requirements
• Ability to assess home environment for safety issues, including fall risk
• Ability to create successful, professional partnerships with Providers and staff
• Excellent verbal communication skills, electronic clinical management system documentation skills
• Proficient in Microsoft Office, Internet/Web Navigation
• Strong prioritization and organizational skills
• Ability to adapt cultural practices/sensitivities to the application of interventions
• Ability to assess member health status, social needs, cultural beliefs, consistent with state licensure scope of practice requirements
• Must be able to prioritize and target interventions, consistent with state licensure scope of practice requirements designed to support long term behavioral change.
