The Field Case Manager is responsible for all aspects of case management including patient assessment, education, coordination of care and integrating other health team disciplines, supervision of nursing staff, development of Care Plan, discharge planning, participates in case management conferences, and provides effective, accurate, and timely communication and documentation. This individual reports to the Zone Manager.

Associate Degree
College Degree Preferred:

  Specific Courses Required:
Previous home health case management experience. Graduate of an accredited Registered Nurse Program. Home Health expeience and Medical Terminology.

B. CERTIFICATION/LICENSURE/REGISTRATION
  Certifications/Registrations:
Valid State of Florida Driver's License and Current Automobile Insurance Policy.
CPR Certification.
  Licensures:
State of Florida - Registered Nurse (RN) License.
C. EXPERIENCE
  Years of Experience:
1-2 Years
Type of Experience:
Registered nursing experience.
D. NECESSARY SKILLS
  Must have case management experience, excellent verbal and written communication skills and communicate clearly and respectfully to patients. Nursing (RN) skills required with the ability to work under stress/pressure and must have accuracy and attention to detail. Must be multi-task oriented and have work prioritization and follow-through skills. Works independently and accurately with minimal supervision. Must be proficient in various computer programs.
