OVERVIEW: 

The Case Manager are station within the THPP and THPFC houses. The Case Manager provides direct support and case management services to assist residents with meeting individualized goals while connecting them with resources in the community.

  • Residential Counselor – THPP & THPFC.

 

QUALIFICATIONS:

Required

  1. Bachelor’s Degree in Health/Human Services field and/or two years full-time equivalent experience in the mental health field
  2. Experience working with Transition-Aged Youth (TAY)
  3. Knowledge of Microsoft Offices applications
  4. Demonstrated skills in case work, group work, and case coordination
  5. Understanding of transformational care planning and targeted case management principles of service delivery
  6. Excellent interpersonal skills (verbal and written) with demonstrated leadership in facilitating and linking services among a diverse variety of mental health programs
  7. Ability to maintain a positive perspective in the treatment and care of emotionally disturbed youth/young adults
  8. Strong knowledge base with Medi-Cal documentation standards.
  9. Demonstrated a strong sense of time management skills
  10. Proven excellent writing and verbal communication skills
  11. Possesses the ability to motivate and maintain effective working relationships with staff and partners

 

Preferred

  1. Master’s degree in a social work or related human service field
  2. Experience working in human services or the mental health field is strongly desired with community-based and/or school-based programs
  3. Minimum 5 years’ experience engaging with Transition Aged Youth (TAY)
  4. Bilingual / bicultural with Spanish fluency
  5. Experience working in a residential setting

 

RESPONSIBILITIES INCLUDE (but not limited to):

  1. Interviews program applicants and provides input into the intake process
  2. Utilize a variety of approaches to establish and maintain a relationship with youth.
  3. Manage a caseload of up to 8 TAY youth.  Maintain individual contact with each youth at least weekly to discuss goals and track progress towards goals.  Work with youth to understand any medications that they are prescribed, helping them learn how to refill prescriptions as needed. 
  4. Maintain ongoing contact with outside persons/entities such as: natural supports, family, referring agency/case carrying agency, ILP, other and community resources. Participate in relevant team meetings such as: Family Support Team meetings, TDMs, and emancipation conferences.  Serve as a representative of UCG and liaison with these parties. 
  5. Support the goals outlined in the Transition Independent Living Plan and work with youth to identify additional goals and tasks as needed. 
  6. Provide on-call back-up on assigned weekends or as necessary during crisis.
  7. Responsible for completing reports, notes and Medi-Cal paperwork in a timely manner. Responsible for inputting data into the client database accordingly.
  8. Facilitate house meetings for youth and staff meetings accordingly.
  9. Establish and monitor budget for house/residents.
  10. Oversee Residential Counselors assigned to work in the THPP program.  This includes interviewing, training, discipline, supervision, scheduling and development of staff.
  11. Attend all regular staff meets and trainings (some meeing and trainings may not fall on employee’s regular schedule). 
  12. Ensure that the facility/program is in compliance with regulations (such as OSHA, HIPAA, CCL, UCG Policy and Procedures among others) as well as with our contract/MOU for THPP.
  13. Ensure records/documents/paperwork is up to date and maintained on site.
  14. Provide transportation as needed when it fits within the resident’s goals.
  15. Employees are responsible to find new resources within the community to support the clients for independency
  16. Other duties as assigned.
