OVERVIEW: 

The Coordinator will serve ILP eligible youth/young adults as a teacher/mentor/guide in order to assist them with their Independent Living Skills needs.

Pay $20.50-$22 hour

QUALIFICATIONS:

        Required

  1. Bachelor’s degree in Behavioral or Social Sciences required.
  2. Minimum of 1-3 years working with youth/young adults.
  3. Knowledge/skills/abilities in transition age youth unique needs and services.
  4. Must meet Community Care Licensing Requirements for employment.

 

RESPONSIBILITIES INCLUDE (but not limited to):

  1. Field internal and external referrals for youth to be engaged with the ILP program.
  2. Complete necessary documentation to include: assessment, intake, generating reports (monthly, quarterly, and annual), meeting write-ups, tracking outcomes, data entry into computerized charting system, keeping paper-based charts,
  3. Provide service activities to both in-care and emancipated youth, consistent with their TILP in a 1:1 setting, group setting, or both.
  4. Provide ancillary individual case management for the youth focusing on their life skill needs to include: education, employment, life skills.  This case management is not meant to take the place of any other case management they are/should be receiving (such as: from group home agency, or THP provider), but as a supplement.
  5. Be available and attend youths’ child family team meetings and emancipation conferences in order to provide feedback, support and resources related to the youth’s upcoming transition into adulthood.
  6. Attend ILP meetings and conferences.
  7. Provide needed transportation to enable a youth to participate in ILP related events if it fits within their TILP.
  8. Assist and guide youth in locating/navigating educational opportunities, vocational training, and employment opportunities.
  9. Distribute financial incentives/assistance to the youth in the form of incentives and/or emancipated youth stipend.  Maintain records of all distributions.
  10. Collaborate and coordinate with other agencies and community resources in order to better service the youth.
  11. Other duties as assigned.
